Athletic Medical Forms & Handbooks
A new Preparticipation Physical Evaluation is required each school year for students interested in athletics and/or cheerleading at the middle school level; and for high school students involved in athletics, band, dance, drill team, cheer, and/or ballet folklorico. This form must be fully completed and received by the Bastrop or Cedar Creek Athletic Training Department before your student is able to participate in any of the above listed activities at their grade level. Per BISD Policy, all athletic physicals must be completed after April 25th of the current year to be valid for the following school year.
