Bastrop ISD and Seton Health Plan have teamed up to be able to offer students immunizations during the annual Back to School Bash. This year the event will be held on Saturday, August 23 from 8:30-10:30 a.m. at the BISD Memorial Stadium located at 755 Highway 21, Cedar Creek. In order to receive immunizations, students must have their immunization record and have a parent or guardian present while they receive their shots.

Exemptions:
The law allows (a) physicians to write a statement stating that the vaccine(s) required is medically contraindicated or poses a significant risk to the health and well-being of the child or any member of the child's household, and (b) parents/guardians to choose an exemption from immunization requirements for reasons of conscience, including a religious belief.

Instructions for the affidavit to be signed by parents/guardians choosing the exemption for reasons of conscience, including a religious belief can be found here.

To request an exemption online, click here.  To print out an exemption form, click here.

For children needing medical exemptions, a written statement by the physician should be submitted to the school or child-care facility.

Medication Release

In order to administer medication to your child, we must have:

  1. Your signed permission
  2. All medication must be in the labeled original container
  3. For medication that needs to be taken at home and at school, ask your pharmacist to fix two bottles for you so that you may leave one at school (children do forget to pick up medication to take home.)
