Exemptions:
The law allows (a) physicians to write a statement stating that the vaccine(s) required is medically contraindicated or poses a significant risk to the health and well-being of the child or any member of the child's household, and (b) parents/guardians to choose an exemption from immunization requirements for reasons of conscience, including a religious belief.

For children needing medical exemptions, a written statement by the physician should be submitted to the school or child-care facility.

Medication Release

In order to administer medication to your child, we must have:

  1. Your signed permission
  2. All medication must be in the labeled original container
  3. For medication that needs to be taken at home and at school, ask your pharmacist to fix two bottles for you so that you may leave one at school (children do forget to pick up medication to take home.)
